What Are Peptides — And Are They Safe to Use?
This is a popular question heard in our office. You’ve probably heard the word "peptides" popping up everywhere — on social media, in wellness clinics, and maybe even from a friend who swears they've changed their life. But what exactly are peptides, which ones are backed by real science, and which ones should you be cautious about? These are important things to know in a world of misinformation.
What Is a Peptide?
A peptide is simply a short chain of amino acids — the same building blocks that make up proteins. While proteins are long chains (typically more than 50 amino acids), peptides are shorter, usually between 2 and 50 amino acids linked together. Your body naturally produces many peptides that serve as hormones, signaling molecules, and chemical messengers. Insulin, for example, is a peptide hormone that regulates blood sugar. Oxytocin — sometimes called the "love hormone" — is another naturally occurring peptide.
Because peptides are smaller than proteins, they can be designed to interact very precisely with specific targets in the body. This makes them attractive as medications. In fact, peptide-based drugs have been used in medicine for over 100 years, starting with insulin in the 1920s.
FDA-Approved Peptides: The Ones Backed by Science
When a peptide drug is FDA-approved, it means it has gone through rigorous clinical trials proving it is both safe and effective for a specific medical condition. There are currently more than 80 FDA-approved peptide drugs on the market. Here are some well-known examples:
• Insulin (various forms) — The original peptide drug, used since the 1920s to manage diabetes.
• Semaglutide (Ozempic, Wegovy, Rybelsus) — A GLP-1 receptor agonist peptide approved for type 2 diabetes, weight management, cardiovascular risk reduction, and chronic kidney disease in diabetic patients.
• Tirzepatide (Mounjaro, Zepbound) — A dual GLP-1/GIP receptor agonist peptide approved for type 2 diabetes, weight management, and obstructive sleep apnea in adults with obesity.
• Oxytocin (Pitocin) — Used to induce labor and control postpartum bleeding.
• Octreotide (Sandostatin) — A somatostatin analog used for conditions like acromegaly and certain tumors.
• Bremelanotide (Vyleesi) — Approved for hypoactive sexual desire disorder in premenopausal women.
These medications were developed under strict manufacturing standards, tested in thousands of patients, and are monitored for safety after approval. When your doctor prescribes one of these, you can be confident in what you're getting.
The "Gray Market" Peptides: What's Being Sold Without Approval
Here's where things get concerning. A growing number of peptides are being sold online, for profit, at wellness clinics, and through social media influencers — even though they have never been approved by the FDA for any medical use. These are sometimes called "gray market" peptides because they exist in a regulatory gray zone, often marketed as "research chemicals" or "for research use only" to skirt regulations.
Some of the most commonly marketed unapproved peptides include:
• BPC-157 (Body Protection Compound-157) — Promoted for gut healing, joint repair, tendon recovery, and general "tissue protection." While animal studies show some interesting results, there is virtually no reliable human data proving it works or is safe. It was originally isolated from gastric secretions — not from food — and hundreds of thousands of people are injecting it based on social media hype alone.
• TB-500 / Thymosin Beta-4 — Marketed for wound healing, muscle recovery, and reducing inflammation. Again, the evidence comes almost entirely from animal studies. Both are banned by the World Anti-Doping Agency (WADA) in competitive sports.
• CJC-1295 and Ipamorelin — Often sold together as a "growth hormone booster" stack, promoted for anti-aging, fat loss, and muscle building. These peptides stimulate growth hormone release, but their safety in the doses and combinations people are using has never been studied in humans.
• AOD-9604 — A fragment of human growth hormone marketed for fat loss. It failed to demonstrate efficacy in clinical trials for obesity and was never approved.
• Melanotan II — Promoted for skin tanning and sexual enhancement. It carries risks including nausea, facial flushing, and potential effects on moles and skin lesions.
The Claims vs. The Reality
Sellers and influencers make bold claims about these peptides: "heal your gut," "reverse aging," "burn fat while you sleep," "recover from injuries twice as fast." Here's what the science actually says:
• Most evidence comes only from animal studies. Rats and mice are not humans. Many promising animal findings never translate to safe or effective treatments in people.
• There have been no meaningful clinical trials. For the vast majority of gray-market peptides, there are no large, well-designed human studies proving they work for the conditions they're marketed for.
• Dosing is guesswork. Because these peptides haven't been through clinical trials, no one knows the right dose, how often to take them, or how long is safe.
• "Stacking" is untested. Many users combine multiple peptides at once. The safety of these combinations has never been studied.
Why We Urge Caution
Beyond the lack of proven benefits, there are real safety concerns with unapproved peptides:
1. You don't know what's in the vial. Laboratory analyses of gray-market peptide products have found alarming problems: purity levels as low as 5%, the wrong ingredient entirely, incorrect doses, and contamination with toxic heavy metals including arsenic (at levels up to 10 times the safety limit) and lead. These products are manufactured outside of regulated quality systems, so there is no guarantee of what you're actually injecting.
2. Potential for real harm. Peptides that stimulate growth hormone can cause hormonal imbalances, insulin resistance, fluid retention, joint pain, and potentially increase the risk of abnormal cell growth. Injecting unsterile products carries risks of infection.
3. The placebo effect is powerful. Research suggests that much of the benefit people report from gray-market peptides may be attributable to the placebo effect — especially when reinforced by social media communities where users share positive experiences and encourage each other.
4. The regulatory landscape is shifting — but not because of new safety data. While some regulatory changes may make certain peptides more available in the future, patient safety organizations have pointed out that these changes are not based on new scientific evidence of safety or effectiveness.
The Bottom Line
Peptides are a legitimate and exciting area of medicine. FDA-approved peptide drugs like semaglutide and tirzepatide are transforming the treatment of diabetes and obesity based on robust clinical evidence. But the unapproved peptides flooding the internet and wellness clinics are a different story entirely.
